| To make the Application process as user-friendly as possible for you, please read the following information carefully to ensure a complete application. If you have any questions, do not hesitate to contact the school office at 781-444-4459. - Download all Application Forms (See PDF files below) or obtain an Application Packet from the school office
- Return completed forms to the school office ATTN: Admissions
- Be sure to include all required written documentation (see below)
Students entering the school for the first time are required to provide the following: - A copy of the child’s birth certificate
- A copy of the child’s baptismal record (if applicable)
- All current immunization records. All new students must have a recent physical examination, documented within 12 months prior to entering school.
- Students entering from another school are required to provide their academic records, including past report cards and standardized test scores.
- A copy of the child’s Individual Education Plan (if applicable)
- Other information may be requested from the student’s current school. A screening test will be administered to each new applicant.
